Observation standard
Five rules before you submit.
State the behaviour
Record words, actions, procedural events or visible changes that can be dated and reviewed.
Separate source from interpretation
Say whether you observed it directly, received a report, or are referencing an operational record.
Avoid diagnosis
Do not label someone as unstable, radicalised, paranoid, dangerous or mentally ill.
Exclude identity
Religion, ethnicity, nationality, politics and other protected or sensitive traits are not signal inputs.
Escalate immediate danger directly
Urgent threats follow the organisation’s emergency procedure; this form does not replace emergency response.
